Christopher I. Li, M.D., Ph.D., and colleagues at Fred Hutchinson Cancer Research Center report these findings in the November issue of Cancer Epidemiology, Biomarkers and Prevention.</p>
<p>&#8220;We found that, overall, women who had a history of migraines had a 30 percent lower risk of breast cancer compared to women who did not have a history of such headaches,&#8221; said Li, a breast-cancer epidemiologist and associate member of the Hutchinson Center&#8217;s Public Health Sciences Division.</p>
<p style="text-align: justify;">In particular, migraine history appeared to reduce the risk of the most common subtypes of breast cancer: those that are estrogen-receptor and/or progesterone-receptor positive. Such tumors have estrogen and/or progesterone receptors, or docking sites, on the surface of their cells, which makes them more responsive to hormone-blocking drugs than tumors that lack such receptors.</p>
<p>The biological mechanism behind the association between migraines and breast cancer is not fully known, but Li and colleagues suspect that it has to do with fluctuations in levels of circulating hormones.<br />
<span id="more-19"></span>&#8220;Migraines seem to have a hormonal component in that they occur more frequently in women than in men, and some of their known triggers are associated with hormones,&#8221; Li said. &#8220;For example, women who take oral contraceptives - three weeks of active pills and one week of inactive pills to trigger menstruation - tend to suffer more migraines during their hormone-free week,&#8221; he said. Conversely, pregnancy - a high-estrogen state - is associated with a significant decrease in migraines. &#8220;By the third trimester of pregnancy, 80 percent of migraine sufferers do not have these episodes,&#8221; he said. Estrogen is known to stimulate the growth of hormonally sensitive breast cancer.</p>
<p>While this study represents the first of its kind to look at a potential connection between migraines and breast cancer, Li and colleagues have data from two other studies that in preliminary analyses appear to confirm these findings, he said. &#8220;While these results need to be interpreted with caution, they point to a possible new factor that may be related to breast-cancer risk. This gives us a new avenue to explore the biology behind risk reduction. Hopefully this could help stimulate other ideas and extend what we know about the biology of the disease.&#8221;</p>
<p>For the study, the researchers combined data from two population-based, case-control studies of 3,412 Seattle-area postmenopausal women, 1,938 of whom had been diagnosed with breast cancer and 1,474 of whom had no history of breast cancer, who served as a comparison group. Information on migraine history was based on self-report and was limited to migraines that had been diagnosed by a physician or other health professional.</p>
<p>The National Cancer Institute funded the research, which also involved researchers from the Hutchinson Center&#8217;s Human Biology Division and the University of Washington School of Medicine Department of Neurology.</p>

The results, which suggest a possible role for vitamin D in weight loss, were presented at The Endocrine Society&#8217;s 91st Annual Meeting in Washington, D.C.</p>
<p>&#8220;Vitamin D deficiency is associated with obesity, but it is not clear if inadequate vitamin D causes obesity or the other way around,&#8221; said the study&#8217;s lead author, Shalamar Sibley, MD, MPH, an assistant professor of medicine at the University of Minnesota.</p>
<p>In this study, the authors attempted to determine whether baseline vitamin D levels before calorie restriction affect subsequent weight loss. They measured circulating blood levels of vitamin D in 38 overweight men and women before and after the subjects followed a diet plan for 11 weeks consisting of 750 calories a day fewer than their estimated total needs. Subjects also had their fat distribution measured with DXA (bone densitometry) scans.<span id="more-16"></span>On average, subjects had vitamin D levels that many experts would consider to be in the insufficient range, according to Sibley. However, the authors found that baseline, or pre-diet, vitamin D levels predicted weight loss in a linear relationship. For every increase of 1 ng/mL in level of 25-hydroxycholecalciferol - the precursor form of vitamin D and a commonly used indicator of vitamin D status - subjects ended up losing almost a half pound (0.196 kg) more on their calorie-restricted diet. For each 1-ng/mL increase in the active or &#8220;hormonal&#8221; form of vitamin D (1,25-dihydroxycholecalciferol), subjects lost nearly one-quarter pound (0.107 kg) more.</p>
<p>Additionally, higher baseline vitamin D levels (both the precursor and active forms) predicted greater loss of abdominal fat.</p>
<p>&#8220;Our results suggest the possibility that the addition of vitamin D to a reduced-calorie diet will lead to better weight loss,&#8221; Sibley said.</p>
<p>She cautioned, however, that more research is needed. &#8220;Our findings,&#8221; she said, &#8220;need to be followed up by the right kind of controlled clinical trial to determine if there is a role for vitamin D supplementation in helping people lose weight when they attempt to cut back on what they eat.&#8221;</p>

			<content:encoded><![CDATA[<p style="text-align: justify;">Surgeons once recommended weight-loss surgery only for severely obese patients who failed to drop pounds with conventional weight-loss methods, but a review now finds that bariatric surgery helps the moderately obese lose more weight, too.</p>
<p>&#8220;Until recently, only people with severe obesity with a body mass index (BMI) greater than 40 were considered for bariatric surgery,&#8221; said review author Jill Colquitt, Ph.D.</p>
<p>But studies, such as those included in this review, now examine the effects of surgery on people with a BMI of 30 to 40 who have diseases such as type 2 diabetes or hypertension that potentially could improve, said Colquitt, a senior research fellow at the University of Southampton, in England.</p>
<p>&#8220;We see a wide range of patients who consider surgery. The majority are people that attempted medical weight loss for years and decades without success, and they have an intimate understanding of what morbid obesity means to them in their life. They&#8217;re looking for a therapy that can give them some help,&#8221; said Peter Hallowell, M.D., an assistant professor of surgery at the University of Virginia. He has no affiliation with the review.</p>
<p>In the new review the third update of a 2002 review researchers led by Colquitt examined 26 previously published studies on bariatric surgery involving 5,766 patients. Five of the included trials took place in the United States.<span id="more-13"></span>Six studies compared bariatric surgery outcomes to those from conventional weight loss management. Twenty studies compared different bariatric surgery procedures.</p>
<p>The review appears in the latest issue of The Cochrane Library, a publication of The Cochrane Collaboration, an international organization that evaluates medical research. Systematic reviews like this one draw evidence-based conclusions about medical practice after considering both the content and quality of existing medical trials on a topic.</p>
<p>The conclusions of the new review were broadly similar to previous research, Colquitt said. Researchers found that weight reduction surgery in obese patients led to more weight loss than conventional methods, such as dieting and exercise.</p>
<p>However, &#8220;since we conducted the first review, we made changes to the inclusion criteria to include people with a lower threshold of obesity,&#8221; Colquitt said.</p>
<p>Specifically, two trials examined people of moderate obesity with BMIs between 30 and 40 who had weight-related conditions such as type 2 diabetes.</p>
<p>In these patients, weight reduction was greater two years after surgery and conditions like diabetes and metabolic syndrome improved, compared to those in the conventional management group.</p>
<p>For example, one study found that moderately obese people who received weight-loss surgery dropped 87.2 percent of excess weight. In comparison, those who used drugs, diet and exercise only lost 21.8 percent of excess weight.</p>
<p>The review evidence also suggested that the type of surgery a patient had affected their weight-loss results. In gastric bypass, surgeons make the stomach smaller and shorten the length of the small intestine, whereas gastric banding involves using silicone bands that the physician can adjust to reduce the stomach&#8217;s size.</p>
<p>Researchers found that gastric bypass led to greater weight loss than vertical banded gastroplasty or adjustable gastric banding. The results were similar for gastric bypass and two techniques called isolated sleeve gastrectomy and banded gastric bypass.</p>
<p>However, it is not possible to draw any conclusions because of the small number of studies comparing each procedure and the risk of bias in some of the trials, Colquitt said.</p>
<p>Some complications from surgery did occur, such as pulmonary embolism and post-operative death. Most studies had no deaths and those that did had one or two.</p>
<p>Although they aimed to update the review with information about bariatric surgery in patients younger than 18 years, researchers found no studies that compared surgery with conventional management in this group of patients, so they could not comment, Colquitt said. They also excluded older trials that examined surgical techniques no longer used &#8220;to keep the review as relevant and helpful as possible,&#8221; Colquitt said.</p>
<p>&#8220;The frontline question is, &#8216;Is surgery better for patients with lower levels of obesity?&#8217; Those are areas of research that are just beginning to come to light,&#8221; Hallowell said.</p>
<p>&#8220;Their findings are very important. In the small number of randomized controlled trials to look at, there&#8217;s clear evidence that surgery is better than not having surgery,&#8221; Hallowell said.</p>
<p>&#8220;There are risks to surgery we don&#8217;t want to minimize that to any degree but the health benefits noted in these studies certainly outweigh the risks for patients who undergo it,&#8221; Hallowell said.</p>

			<content:encoded><![CDATA[<p style="text-align: justify;">Yesterday&#8217;s 19th European Congress of Clinical Microbiology and Infectious Diseases (ECCMID) conference in Helsinki reported a significant addition to the dwindling number of drugs in the fight against the hospital &#8216;Superbug&#8217;, Methicillin Resistant Staphylococcus aureus (MRSA).</p>
<p style="text-align: justify;">UK-based pharmaceutical company Destiny Pharma presented research into the rapid anti-bacterial activity of its latest platform of drugs against Staphylococcus aureus on the second day of the conference. Whilst the company&#8217;s lead anti-microbial XF-73 continues on course within Phase I/IIa clinical development, Destiny has now demonstrated the novel anti-bacterial mechanism of action of DPD-207. This is an example of its new platform of Metallo-Porphyrin (MP) drugs which has been proven to act in a similar manner to the XF drug platform.<span id="more-8"></span>The XF drugs:</p>
<p style="text-align: justify;">1. Kill bacteria extremely quickly (much faster than traditional antibiotics)<br />
2. Bacteria such as MRSA seem unable to become resistant to XF/MP drug action (unlike many antibiotics to which the Superbug can rapidly become resistant)<br />
3. Kill bacteria within biofilms (the protective &#8216;jelly&#8217; bacteria produce around themselves, which acts as a protective barrier against antibiotics)</p>
<p style="text-align: justify;">The MP drugs appear to share many of the same revolutionary and highly advantageous anti-bacterial properties of the XF drugs.</p>
<p style="text-align: justify;">The MP drug platform represents a significant expansion of the Destiny Pharma drug portfolio.</p>

<p style="text-align: justify;">In the USA around 126,000 people are hospitalised each year with MRSA-related infections resulting in 19,000 fatalities. In the UK, MRSA causes over 6,000 serious bloodstream infections each year and results in at least 1,600 deaths. Healthcare-associated infections (HCAIs) predominantly caused by MRSA, are estimated to cost the NHS over £1 billion every year and the US economy more than $30 billion</p>

<p style="text-align: justify;">The XF compounds are bactericidal rather than bacteriostatic; killing the bacteria instead of just limiting population growth. These compounds have a unique structure based on a porphyrin core and are chemically distinct from all existing antimicrobial therapies.</p>
<p style="text-align: justify;">In laboratory studies, the XFs are active against a wide variety of clinically important gram-positive bacteria, including the most common and problematic MRSA strains and two clinical isolates of Clostridium difficile.</p>

			<content:encoded><![CDATA[<p style="text-align: justify;">Spring is in the air, and so is a lot of annoying pollen. That means many North Carolinians are sniffling and sneezing as their eyes water and throats scratch. It&#8217;s estimated that 1 in 5 people suffers from allergic rhinitis.</p>
<p style="text-align: justify;">&#8220;The most common allergens right now are tree pollens,&#8221; says Dr. Maya Jerath, an assistant professor in the Division of Rheumatology, Allergy and Immunology at the University of North Carolina at Chapel Hill School of Medicine, director of the Adult Allergy Clinic there and member of the UNC Thurston Arthritis Research Center.</p>
<p style="text-align: justify;">But don&#8217;t blame pine pollen, which covers most of the Old North State with a green dust this time of year.</p>
<p style="text-align: justify;">&#8220;People attribute their allergies to pine pollen because it&#8217;s visible, but it&#8217;s actually too large to cause allergies,&#8221; Jerath explains. &#8220;It&#8217;s the other trees blooming at the same time like maple, oak and birch.&#8221;</p>
<p style="text-align: justify;">Allergy season lasts from late February to late April, but most people suffer only for a couple of weeks while the tree they&#8217;re sensitive to is in bloom. Folks with multiple allergies may experience symptoms longer, however.</p>
<p style="text-align: justify;">Most allergy sufferers can find relief from an over-the-counter antihistamine. &#8220;Medicines containing diphenhydramine like Benadryl are very effective,&#8221; Jerath says. &#8220;But, these act for a short period of time, and they can make you sleepy.&#8221; Longer-lasting alternatives are loratidine (Claritin) or cetirizine (Zyrtec), which typically don&#8217;t cause drowsiness and come in once-a-day formulas. <span id="more-5"></span>If you have short-lived allergies for only two or three weeks a year, it&#8217;s fine to treat them symptomatically. &#8220;But if the medicine is not sufficient to keep you from suffering, or if you cannot tolerate the medicine, it&#8217;s time to go to the doctor,&#8221; Jerath says. &#8220;There are many prescription medications that work well for allergies. Similarly, if your symptoms last for more than a season, you can see a doctor to find out what you might be allergic to, and to see if you&#8217;re a candidate for immunotherapy a treatment that aims to cure allergies.&#8221;</p>
<p style="text-align: justify;">Children with allergies and asthma may also benefit from an allergy evaluation. Controlling allergy symptoms can help asthma control and also can prevent some of the common complications of untreated allergies like sinusitis and ear infections.</p>
<p style="text-align: justify;">Some sufferers seek relief with supplements like Vitamin C or zinc. &#8220;These supplements may help with colds, but there are no studies showing they work for allergic rhinitis,&#8221; Jerath notes. &#8220;However, nasal saline rinses can be helpful in mitigating symptoms because they minimize exposure by clearing out any allergens that might be present. In addition, there are a few small studies that show regular use of these rinses can change the cells lining your nose making it less prone to inflammation, which creates that stuffy feeling.&#8221;</p>
<p style="text-align: justify;">The best way to reduce the impact of seasonal allergies is to avoid exposure, but, Jerath admits, &#8220;It&#8217;s unrealistic to tell people not to go outside. Pollen&#8217;s not something you can turn off, but if you know which pollen you&#8217;re sensitive to, you can minimize your time outside during the season it is in the air and keep your windows closed.&#8221;</p>

			<content:encoded><![CDATA[<p style="text-align: justify;">A Sydney allergy practice has found an &#8216;overwhelming majority&#8217; of a group of patients who developed a rare allergy to red meat had previously had an adverse reaction to tick bites.</p>
<p style="text-align: justify;">Clinical Immunologists, Drs Sheryl van Nunen and Suran Fernando and colleagues, of Royal North Shore Hospital, found that 24 of 25 patients who presented with a history of allergic reaction to red meat also reported large local reactions to tick bites.</p>
<p style="text-align: justify;">&#8220;Seventeen of the 25 had a severe reaction to the bite, such as tongue swelling, throat constriction or shortness of breath,&#8221; Dr Fernando said.<span id="more-3"></span>Patients then reported that they had previously been bitten by a tick, resulting in a painful, itchy lesion greater than 50mm that lasted at least a week.</p>
<p style="text-align: justify;">&#8220;All of the patients lived in Sydney&#8217;s northern beaches area, which is endemically infested with several tick species.</p>
<p style="text-align: justify;">They all described the culprit as a hard-bodied tick shaped like a human fingernail ranging in size from 3 to 10 mm. This is most likely to be Ixodes holocyclus, commonly known as the paralysis tick, which is &#8220;not only the most commonly found species that affects humans in the area, but is also the species most associated with hypersensitivity reactions in humans&#8221;.</p>
<p style="text-align: justify;">The researchers set up a retrospective control, contacting 29 patients from the clinic with allergies to foods other than red meat residing in the same region. All 29 reported a history of tick bites without subsequent reaction.</p>
<p style="text-align: justify;">&#8220;The findings suggest that, in our patient population, the overwhelming majority of people with the relatively rare condition of red meat allergy are preceded by sensitisation to tick bites,&#8221; Dr Fernando said.</p>
<p style="text-align: justify;">Further research is needed, but it could be that &#8220;individuals are sensitised to tick salivary proteins that are cross-reactive with proteins found in various red meats&#8221;.</p>
<p style="text-align: justify;">&#8220;To determine the true prevalence of this cross-reactivity, a prospective study could examine consecutive patients who are bitten by ticks (with and without reactions) for the subsequent development of red meat allergy.&#8221;</p>
